Anthony - some of the Rogers Commission report is freely available.
There is also a report from a medical professional who determined that the crew members were alive, albeit probably unconscious at least until they hit the water. Three personal egress air packs had been activated.

A recent report I read said that the Challenger Disaster report could have been used, with minor edits, to describe shortcomings that led to the Columbia disaster - NASA implemented few of the recommendations. I think lessons were learned, but little was done to fix them.
May they all continue to rest in peace.

It was declassified alright. I watched it as an episode on "NASA: 50 Years of Explorations" 6 disc DVD set (from Michael O'Connell). It broke down the final 6 seconds or so into a 30 minute report, milisecond by millisecond.

Are you sure the report was declassified? I remember reading that the report is "sealed" for 50 years (or something similar). Perhaps I am thinking about parts of the report including the issue if they were still alive when the explosion occurred and the cabin was in free-fall.
