Keith,
I very carefully used cotton buds to clean a particularly nasty fleck that got on my 300D sensor, its do-able but you need to be very careful or you'll make it worse.

Try using a SpecGrabber (available from camera shops for less than €20) for delicately picking dust and specs from the CCD - it's designed for the purpose. Don't think it'll do much for a smear though.

Keith,
The IR filter is covering the sensor, its a couple of mm thick so there's no chance of actually harming the sensor underneath. Your only fear is scratching the glass. As long as you use a soft lens cloth there is little chance in that happening either. I use 60% distilled water / 40% Isopropyl alchohol, just moistening the cloth enough to do the job.
Just be careful not to drag any hard pieces of dirt across the glass.

Apparently it makes quite a noticable scratching sound. A friend in work managed to ruin a camera this way.
