The ECO (Engine Cut Off) sensors keep failing, making it possible the Shuttle's 3 main engines would cut out before reaching orbit (even before SRB seperation). NASA actually doesn't know why they're failing, and remark that the problem is akin to an electrical fault in a car that seems to fix itself by the time it gets to the mechanics shop.

What I find amazing is that that rocket engine, the F1, is so beautifully made of titanium and other exotic elements and all it had to do was work for 2mins and 30 secs.

Add a cost of about half a million dollars each too... back in 1979!

Earlier than that Seanie - Saturn Vs were launched from 1967 (Apollo 4) - 1973 (Skylab).
